We look forward to serving you as a customer! Below are some documents we’ll need you to complete in order to kick off the process of adding you as a customer.

    • If you are request payment terms such as net 30 or taking product on consignment  – please click the button for “Credit Application” and complete the linked form.
    • If you are not requesting credit terms and plan to pay for orders as they are placed (ie – a “cash” customer) – please click the button for “New Customer Info Form.”

Only one form needs to be completed, however, we cannot provide you with Credit Terms if the Credit Application is not completed, if you are unsure, fill out the Credit Application and discuss with your Sales Rep. 

Please ensure you have attached your tax-exempt certificate - currently we are only able to sell to re-sale customers.

If requesting consignment terms - a consignment agreement will be created and sent to you for signature after credit approval.
